How do I get my passbook to compete?

New Master Boxers must provide two passport size photos and original birth certificate for proof of age along with the membership application and registration fee before their application can be considered complete.
They must also provide the results for their physical examination by their personal physician. They must receive a physical exam each year they are registered.
The doctor also has to provide a letter or form that states the athlete is fit to compete.
Master Boxers must pass an annual medical exam and be declared “fit to box” with the following elements before being registered to box:
(1) No history of diabetes or chest pain.
(2) No history of chronic headaches.
(3) Blood pressure less than 145/90
(4) Graded exercise EKG every five (5) years beginning at age 45.
(5) Normal eye examination by an ophthalmologist every five (5) years.
